History

 

 

 

 

Fostoria Community Hospital received many calls for assistance from caregivers who would ask "I need somewhere to take my husband, who has Alzheimer's disease, so I can run errands or get some rest. I don't want to put him in a nursing home, yet." 

DayBreak was born on April 17, 2000, from grants from the following:

1.    Brookdale National Foundation

2.    Cummins Foundation

3.    Greater Fostoria Community Foundation

4.    Fostoria Rotary

5.    Henry Geary Community Initiative Grant

6.    In kind assistance from Fostoria Community Hospital and the Good Shepherd Home.

You can find DayBreak on the lower level of the Good Shepherd Home in Fostoria, Ohio. The program assists caregivers on Mondays, Wednesdays, and Fridays from 8:30 a.m. until 2:30 p.m.

Participants of DayBreak range from early to mid-stages of memory loss whether the illness is related to Alzheimer's disease, stroke, or other dementias.

Support

 

 

 

DayBreak is a nonprofit 501 (c) 3 organization and is a United Way agency.  Financial support comes from fund raising activities such as the annual spaghetti dinner and craft bazaar.  Grants and support monies come from the United Way and various grants each year.

Transportation

 

 

 

SCAT

Arrangements can be made with the Seneca Commission on Aging Transportation (SCAT) for clients who are in need of a ride.  A nominal fee is charged for one-way or round trips.  You can contact SCAT at 419-448-0001.  SCAT is located in Tiffin.

Communi Van

Is an affordable, accessible transportation to medical services within Fostoria, Ohio.  This is not emergency transportation.  Communi Van is open Monday through Friday between 8 a.m. and 4 p.m.  Trips are scheduled on a first-come, first-served basis.  At least a 24 hour in advance scheduling notice should be made.  Communi Van telephone number is 419-436-6634. The Communi Vans are provided by Fostoria Community Hospital. 

Commission on Aging

Provides transportation to all seniors in Fostoria. Free service. Need at least 24 hours advance notice. Call 436-0436.
